How do I send my order to this provider?

The order can be faxed to (620)-665-2069 or brought to the appointment. Discuss the order specifics with the provider during scheduling.

Will my results be sent back to the ordering provider?

The results will be sent back to the ordering provider if the order explicitly requests it. Ask your ordering provider to include instructions on where to send your results. If your results are not sent, please contact the provider who performed your procedure.

Your purchase includes the following services:

• Facility fee – technical (equipment) fee for the service
• Physician fee – physician interpretation fee

This HutchCares bundled price includes the cost of your procedure and the fees listed above. These fees are for the services most frequently packaged together with this procedure. Any services provided at the time of treatment that are not listed here will not be covered in your purchase.

If you require additional treatment at the time of service, you may incur additional costs for the following reasons:

• During or before your procedure, your doctor may deem it necessary to order additional tests.
• Your procedure may result in a need for further diagnostic testing.
• Your provider may choose to perform additional procedures at the time of service
• Additional specialists may be required to examine or participate in your procedure.
• Your procedure may require separately billable durable medical equipment (DME).
